In order to be granted access to medcap.unito.it requester user should:

1. attend the online medcap policy course, perform the evaluation test and obtain the Medcap Policy Awareness Certificate (MPAC)
2. download and fill-in the Medcap Head of Department Data Processing Authorization form (MHDDPA)
3. get the form digitally signed by the Head of Department. The document can be signed with the u-sign online digital signature service. Please use "PADES grafico" signature; signature field is labelled HDSignature.
4. Fill-in the online Medcap Account Request Form (MAR): user would be requested to upload MPAC and MHDDPA.
